Qualifying employees may be eligible for the following:
Each employee shall be provided at the District’s expense a dental insurance program at least equivalent to the coverage as amended by the insurance specifications bid in March of 1985, provided the program of coverage remains available from the District’s carrier or a comparable group insurance carrier. Family dental insurance shall be available for employees to purchase subject to carrier approval.
Each employee shall be covered by a term life insurance program paid for by the District which provides a minimum death benefit of $10,000.00. Such Insurances shall also provide double indemnity for accidental death. The employee at his option may, through payroll deduction, subscribe to additional life insurance coverage for amounts no less than $4,000.00. Additional amounts may be available depending on the carrier. Employees hired before 8/1/2007 will be allowed to maintain their current levels of voluntary life insurance.
Each employee shall be provided at the District’s expense a long-term disability insurance protection program at least equivalent to the coverage as amended by the insurance specifications bid in March of 1985, provided the program of coverage remains available from the District’s carrier or a comparable group insurance carrier.
A Comprehensive Major Medical program will be provided by the District with co-payments for office visits in network, deductibles, coinsurance of 10% in network/20% out of network, prescription drug benefits, and out of pocket maximums. The deductible will be waived for physician office services from a preferred provider. (The level of benefits shall be established by Wellmark Blue Cross/Blue Shield.)
Optional health plans shall be available for employee selection with the level of benefits established by the Wellmark Blue Cross/Blue Shield.
These options will be available without additional cost to the District.
Optional HMO health plans shall be available for employee selection with the level of benefits established by the Wellmark Blue Cross/Blue Shield - Select 250, 500, and 750 plans.
Employees who elect to enroll in an optional health plan that costs less than the basic plan (A.1.a.) shall realize the savings as additional salary. The District will make a contribution per month beyond single coverage towards family insurance benefits.
Employees desiring to change from one option to another may do so by contacting the Benefits Coordinator prior to or during the open enrollment period. The open enrollment period is in September of each year.
The District offers two Avesis vision plans for all employees and their immediate families except substitutes and nondistrict coaches. Each plan is voluntary, and available at the employee’s expense
The first plan available provides coverage for an annual eye exam and hardware. The hardware includes lenses every 12 months, frames every 24 months and contact lenses every 12 months. The second plan available provides coverage for the hardware only.
Employees that have the Wellmark Blue Cross Blue Shield Choice health insurance coverage already have coverage for one annual routine eye exam. The monthly premium payments will be payroll deducted on a
Each employee of the District is covered by worker’s compensation if injured on the job. The District agrees to pay each employee the difference between the amount worker’s compensation pays and his normal salary on a per diem basis. The portion which the District pays shall be charged against the employee’s sick leave on a prorated basis.
